CONSENT TO SHARE ELECTRONIC DATA - PATIENT DECISION FORM

As a patient, you may receive care and treatment from a number of places such as your GP practice, hospitals and community services. The document in the link below aims to inform you about how information is kept, and shared by the variousorganisations. It outlines the main differences between the national recordsystem, Care.data, which allows the NHS to use your information for planning and research, and is entirely separate from the data that we share within the NHS in Stockport. This is for the benefit of your health and they are called Stockport Health & Social Care Record and the Summary Care Record (SCR)

Stockport Health & Social Care Record

The current primary care shared record in Stockport is called ‘The Stockport Health Record’ (SHR). It is a local health record which brings together information from different parts of the NHS in Stockport. The integrated care record will now expand across social care making this a more holistic record.

NHS Stockport Clinical Commissioning Group (CCG), Stockport Council, Stockport NHS Foundation Trust, Pennine Care NHS Trust and their partners are committed to a fundamental reformof the way care is delivered. The vision for integrating health and social careis to have one referral and assessment route, one integrated care plan andrecord, one contact point and one pooled budget.

Itshould be emphasised that the record is kept on a secure database that ismaintained by NHS Stockport CCG. The data is never sent to other areas outside the health and social care system. Access to the record is restricted toprofessionals with a duty of care to the person and is only accessed through the secure NHS Network at the point of care.

If an emergency situation arises timely access to your health records and medical history will ensure that the clinician treating you will at a glance have a complete picture of your care in order to make the best decisions about your diagnosis, treatment and care plan. Your integrated health record includes information like test results, medications, allergies andsocial care or mental health information relevant to your medical care.

You can choose whether or not to have a StockportHealth Record. If you choose to have a Stockport Health Record, you do not needto do anything. This will happen automatically. If you choose not to have a Stockport Health Record, please complete the following to record your decision:
